Academic Forms and Procedures
All forms requiring advisor signature or approval for processing must be received before 5:00pm by the advising office on the deadline for the specific form. Please consult the Academic Calendar for the appropriate deadlines. See detailed instructions for each form; some forms are available online and some are available for pickup in the Advising & Career Advising office, Room 342, Hall of Languages. Click on the name of the form to print a form or view a sample accordingly.
Add/Drop
Students in the College of Arts and Sciences can drop courses on MySlice through the Academic/Financial deadline of each semester. After the deadline, students must withdraw if they would like to discontinue their enrollment in any course.
Students may not add classes after the Add Deadline. For extenuating circumstances, see your College Advisor to determine if you may be eligible to submit a Request to Late Add a Course. Students must qualify for College approval first, and the Request will then be reviewed by the Registrar's office.
Appealing for Change in Academic Probation Status
Students who have had one of the following occur may appeal to have their academic probation status removed by the college. Additionally, due to one of the following, the semester in question must also now have a Semester GPA above a 2.0 and must not have occurred more than one year prior to submission of the appeal.
- Completion of an Incomplete.
- Fulfillment of a Missing Grade.
- A formal Grade Change Submitted by the faculty.
If the above conditions are met the college will remove the probation status applied to the semester in question.

Application for Readmission
- Email casmaxread@syr.edu and request the Readmit Application.
- Complete the Readmit Application through the link provided via email prior to the application deadline(s):
Fall Semester deadline: August 1
Spring Semester deadline: December 1
Maymester, Summer Session 1, Combined Summer: May 1
Summer Session 2: June 1
NOTE: International students are encouraged to apply at least 60 days prior to the deadlines provided above, to allow enough time for processing any visa paperwork within home country. - Resolve any holds that you have on your account
- Your application will be reviewed within ten business days from submission, and you will be notified via email.
Audit Application
- Pick up form from 342 Hall of Languages and complete.
- Gather signature from Instructor.
- Sign and submit form to 342 Hall of Languages for Home College Signature.
Declaration of Major
- Log into MySlice.
- Go to eForms to obtain the Major Declaration Form.
- Complete the form.
- You will receive confirmation via email once your form has been approved or denied.
Declaration of Minor
- Print one copy and complete form.
- Gather signature from Minor Coordinator.
- Sign and submit form to 342 Hall of Languages for Home College Signature.
Grade Appeal Process and Documentation

Grading Option - Pass/Fail
NEW ELECTRONIC PROCESS VIA MY SLICE
- Log into MySlice > eForms
- Find the P/F grading option and start your submission. No pdf or paper forms will be processed.
- Once the eForm is submitted, your advisor will review and approve, or get back to you if they have any questions.
Intra University Transfer Application
Complete the IUT Survey document before the first day of the specific semester you wish to start as an Arts & Sciences / Maxwell School student.
Internship Proposal/Agreement
- Print one copy and complete form.
- Gather signatures from Faculty Sponsor AND Department Chair.
- Sign and submit form to 342 Hall of Languages for Home College Signature.
- You will receive a copy in your syr.edu email account in 5-7 business days.
Leave of Absence/University Withdraw
To initiate a request for a leave of absence or withdrawal from the university, contact your College Advisor directly, or visit 342 Hall of Languages during drop-ins to meet with a College Advisor, or email casadvising@syr.edu
Petition for Early Participation in Commencement
Eligibility Criteria:
Candidates who will graduate in December may petition the College of Arts and Sciences for participation in the May Commencement ceremonies. Consideration will be given to those who meet the following criteria:
• Student must develop an academic plan with their Advisor to finish degree by December
1. Please make an appointment with your Advisor via Orange Success, in order to review and discuss your remaining degree requirements and, if eligible for consideration, complete a Petition to the Faculty form to include an academic plan for degree completion.
2. File your Diploma Request in My Slice as a December graduate.
3. Submit your completed Early Participation in Commencement petition to 342 Hall of Languages.
• Dually enrolled (AS/PC and AS/ED) students must obtain approval and signatures from your dual college PRIOR to returning the form to 342 Hall of Languages.
4. You will receive your decision in your syr.edu email account in 5-7 business days.
Bachelor's degree candidates who will complete their course work by August are invited to participate in Commencement in May. An Early Participation petition is not necessary.
NOTE: Participation in convocation and commencement ceremonies does not indicate that degree requirements have been completed. Conferral of degree occurs once all degree requirements are satisfactorily completed.
Petition to Faculty
- Print one copy of form and visit 342 Hall of Languages for instructions (process may vary according to petition).
- You will receive a reviewed copy with a decision in your syr.edu email account in 5-7 business days after you submit.

A COURSE SYLLABUS is needed when:
- You are petitioning for courses to count as HUMANITIES, SOCIAL SCIENCES, OR NATURAL SCIENCES divisions of the A&S Liberal Arts Core that are not currently an approved sequence in the A&S Liberal Arts Core Guidebook.
- You are petitioning for a course to count towards ANY LIBERAL ARTS CORE REQUIREMENT that is not currently listed in the A&S Liberal Arts Core Guidebook.
You need a DEPARTMENTAL SIGNATURE for:
- Major/Minor requirements.
- Waiving a language requirement.
- Taking a graduate level course (as well as the INSTRUCTOR SIGNATURE).
Pre-Health Declaration
Eligibility Criteria:
- Student must have a declared primary major (not ILM)
- Students must have completed 3 full-time semesters of study at Syracuse University by the end of the current term (transfer students – 1 semester of full-time study)
- Have a 3.0 or greater cumulative GPA by the end of the current term
- Login to MySlice
- Go to eForms (Arts & Sciences/Maxwell) and open “Pre-Health Declaration”
- Complete the form.
- You will receive an email regarding the form’s status within 2 business days.
Proposal for Independent Study
- Print one copy and complete form.
- Gather signatures from Faculty Sponsor AND Department Chair.
- Sign and submit form to 342 Hall of Languages for Home College Signature.
- You will receive a copy in your syr.edu email account in 5-7 business days.
Transfer Credit Petitions

Liberal Arts Core Transfer Credit Petition
This form is NOT for Math, Writing, Language courses, or courses for Major/Minor requirements. Please use the departmental transfer credit petition for those courses.

- A course syllabus is required for submission of the petition.
- You will receive a copy of your reviewed petition to your syr.edu account.

Departmental Transfer Credit Petition
Use this form for Math, Writing, Language courses, or classes for majors and minors.

- Download the form, and complete the student section.

- Your major advisor or Minor coordinator
- After the department has reviewed the petition, return it to advising for final evaluation.
- You will receive a copy of your reviewed petition to your syr.edu account.

Transfer to a Different (non-SU) University
For transfer and other applications that require a College Report form to be completed, please submit your forms and request using the link provided below.
Withdraw Petition
- Print one copy and complete Withdrawal Form.
- Sign and submit the copy of the completed form to your assigned college advisor in the College of Arts & Sciences|Maxwell).
Leadership Scholars Stipend Application
Current active undergraduate leadership scholars in Arts and Sciences or Maxwell should use the form below to apply to have their stipend awarded. Stipends are awarded to support students to complete appropriate internships, research experience, study away opportunities, or community engagement activities.
